If you are requesting records from Open Sky, you must provide a signed Release of Information as outlined below for us to process your request.
- For students currently under the age of 18, the Adolescent Release of Information form must be completed and signed by the parent/guardian of the student.
- For students currently over the age of 18, the Young Adult Release of Information form must be completed and signed by the student.
If the records request is from a non-family member, in addition to attaching the signed release, provide an explanation for the reason the records are being requested. An email address is required for the records to be securely sent. If an email address is not available, provide a mailing address. We are unable to fax records. Send the signed Release of Information to recordsrequest@openskywilderness.com. All requests will be processed as soon as possible, however, may take up to 30 days.
